The posterior inferior cerebellar artery (PICA) usually arises from the vertebral artery and follows a complex course around the medulla and cerebellum. After branching off, it winds posteriorly to pass between cranial nerves and wraps around the inferior surface of the cerebellum. It supplies the posteroinferior cerebellar hemisphere, the inferior vermis, and important regions of the medulla. Because of its intimate relationship with the brainstem, occlusion of PICA can cause lateral medullary (Wallenberg) syndrome, characterized by sensory deficits, swallowing difficulties, and balance problems.
